Obituary
Jeremy was born on February 22, 1980 in Edina, MN, to Gregory and Patricia Dzubay. The family moved to Arizona in 1985. He graduated from Centennial High School in Peoria, AZ, then continued his education at Glendale Community College. He really enjoyed working (and playing) with computers, especially the evolving internet. He married AmyLynn Blackmore in May of 2001. He was a loving father to his children Ethan (21 yrs), Elijah (18 yrs), and Emma (15 yrs). He lost both of his legs due to complications from diabetes and passed away after a lengthy hospital stay. Services will be held Friday, May 20th at St. Helen's Catholic Church in Glendale, AZ
